Vilanova Disease (VD) is a variant of erythema nodosum in which a solitary inflammatory plaque appears on the lower leg. It is also called subacute nodular migratory panniculitis and erythema nodosum migrans.
A painless, erythematous nodule enlarging to a plaque on the lower aspect of one shin in a woman is characteristic. The lesion may be chronic with slow expansion. Arthralgias, an elevated ASO titer, and an elevated ESR frequently occur. Hypothyroidism has been associated.
The lesion must be differntiated from cellulitis and lipodermatosclerosis.
Oral potassium iodide is quite effective. IL triamcinolone may be tried for individual small lesions. An NSAID, e.g. indomethacin, may be helpful for the pain and may even clear the lesion. See erythema nodosum for more treatment suggestions.
