Eosinophilic pustular folliculitis of infancy (EPFOI) is a rare disorder of unknown cause. Some patients may have both EPF and infantile acropustulosis. Given the similar histologic appearance, they may represent the same disease in different sites.
Sterile vesicopustules and crusting on the scalp of an infant is characteristic. Papulopustular or erythematous lesions often occur elsewhere. Onset is usually several days to weeks after birth.
A medium to high-potency topical steroid is usually effective. Dapsone has been used but is rarely needed. Cetirizine 2.5 mg/day was used to treat a 27 month old boy with excellent results.
