ZOON'S BALANITIS
Zoon's balanitis (ZB), also known as balanitis plasmocellularis, is an uncommon, benign, idiopathic, inflammatory condition of the penis in older, uncircumcised men.
- A well-demarcated, fixed, red area on the glans penis is seen. Usually the involved skin is constantly covered by foreskin.
- The mean age in one study was 65 years of age. The average duration before presentation was 1-2 years.
Treatment
- A biopsy is critical to exclude SCC in situ (erythroplasia of Queyrat).
- Circumcision cures virtually all patients. However, many elderly patients prefer to avoid circumcision.
- Pulling back the foreskin back and exposing the area more, letting it dry out may help.
- Mupirocin 2% ointment TID for 3-4 months has been effective in several reports.
- A medium-potency topical steroid for 6-8 weeks and then tapered to use as necessary is appropriate.
- In one very interesting report, abrocitinib induced complete remission in one patient within 6 weeks [JAAD Case reports 2024;46;85].
