Mechanism of Action

Avermectins of which ivermectin is one, binds to glutamate-gated chloride ion channels, which are present in invertebrate nerve and muscle cells. This results in paralysis and death of the parasite.


Ivermectin for scabies is usually given 200 ug/kg as a single dose (e.g 18 grams for a 90 kg/198 lb patient) and repeated in 7 days. Ivermectin alone may not be quite as effective as topical permithrin and and may be combined with it, if necessary. It shouldn't be used in pregnancy, in those breastfeeding or in those less than 15 kg where the blood-brain barrier is less effective and the risk of seizures is higher.
